Please be aware that special conditions may apply and that most insurance companies do not allow direct billing if secondary coverage is available. Please contact your insurer directly if you have any questions regarding your coverage. 

If you would like us to bill your insurance directly, please bring the following documents to your appointment: 

• Government issued ID
• Insurance Card
• Credit Card (outstanding amounts that are not covered by your insurance will be charged to your Credit Card)

You also will be required to sign a benefit assignment form.
